In light of the above circumstances, I authorise the filming, recording, capturing and/or photographing of my image or voice, or similar actions and give my consent to:
My image being included, wholly or partially, in their communication media.
My image being disclosed without limitations of place or time, as permitted by Spanish law.
My image being reproduced, distributed and publicised (including dubbing and subtitling into any other language). This right encompasses reproduction for promotions and the rights of reproduction and distribution of my image in all kinds of books, brochures and publications, in both traditional media, such as paper, or electronic media.
Under Organic Law 3/2018, of 5 December, on personal data protection and the guarantee of digital rights, as well as other applicable legislation, the images and personal data obtained from this authorisation will be stored in a file for, which the VALL D'HEBRON UNIVERSITY HOSPITAL, with tax ID no. Q5855029D, located at Passeig Vall Hebron, 119, 08035 Barcelona, is the data controller, and may be processed and used as detailed in this document. The legal basis for such processing under this authorisation is the concerned party's consent, which can be retracted at any time without affecting any legitimate processing conducted up to that point. The concerned party may at any time exercise the rights of access, rectification, erasure, opposition, limitation and portability with the Vall d' Hebron University Hospital. The use of image and voice in relation to the recording, photographic documentation, exploitation and dissemination expressly approved by this authorisation must always uphold the right to personal dignity, considering the exceptions and use restrictions stipulated in Organic Law 1/1982, of 5 May, regarding the civil protection of the right to personal dignity, personal and family privacy and one's own image.
